Understanding shoulder pain
Shoulder pain is pain in or around the shoulder complex, which includes the ball and socket joint, the rotator cuff tendons, the bursa cushioning them, the joint capsule, and the shoulder blade the whole assembly slides across. Any of those can be the source of your symptoms.
The shoulder trades stability for range. It moves further than any other joint from a shallow socket, so the soft tissue around it does most of the work of holding it in place. Four rotator cuff muscles centre the ball in that socket every time you lift the arm, and a bursa sits between the tendons and the bone above so they do not grind. When the shoulder blade stops gliding on the rib cage, the overhead space narrows and the tendons and bursa are compressed on every repetition. Common causes
Rotator cuff tendinopathy
Repeated loading degrades the cuff tendons faster than the body rebuilds them, producing pain on lifting and a dull night ache. It is the most common shoulder source we see.
Shoulder impingement
When the shoulder blade does not glide properly, the space above the cuff narrows and the tendon and bursa are pinched on overhead movement, usually through a specific arc.
Subacromial bursitis
The bursa cushioning the cuff becomes inflamed and thickened, which makes the shoulder painful at rest as well as on movement, particularly at night.
Adhesive capsulitis
The joint capsule thickens and contracts, so range is lost in every direction, including when someone else moves the arm for you. It needs a different plan from tendon pain.
Acromioclavicular joint wear
The small joint on top of the shoulder where the collarbone meets the shoulder blade gives pinpoint pain up top, worse on reaching across the body.
Referred pain from the neck

When to seek care sooner
What to try at home first. For a shoulder sore a couple of weeks with no weakness and no trauma, stop the one movement that provokes the sharpest pain and keep the rest. Total rest is a mistake, because tendons weaken and capsules tighten when a shoulder stops moving. Sleep with a pillow supporting the arm instead of lying on it. Work through gentle range daily, staying below the painful arc. Drop overhead pressing and heavy reaching for a fortnight. If reaching, sleeping and dressing are still restricted after three weeks, get it examined. See someone urgently if the shoulder was injured in a fall or crash and you cannot lift the arm at all, if it looks visibly deformed, if weakness followed a pop or tearing sensation, if there is fever with a hot swollen joint, if you have unexplained weight loss, or if the arm becomes numb, cold or discoloured. Shoulder pain with chest tightness, sweating or breathlessness is an emergency and needs an ambulance, not an appointment.
